Make Your Visit Simple with Our Pre-Appointment Checklist
We ask that you bring in the following to help give us an idea of the current state of your vision and allow us to address any questions or concerns you may have:
- Your most recent pair of glasses and/or contact lens boxes if you have them
- A pair of sunglasses
- A list of any medications you are currently taking
- Medical insurance card
- A list of any questions you may have for Dr. Lillie

Many medications have side effects that impact your vision. Knowing what medications you are taking will help us minimize these and make sure we provide the best plan to give you the clearest, most comfortable vision possible.
Why Medical Insurance?
We should already have all your vision insurance information, so why do we need your medical insurance? This is a great question! We like to keep medical insurance on file in case an emergency or medical need arises – we will already have all the information and can take care of you quickly.
